Chapter 2 - What Lies Beneath

The high-pitched hum of the oscillating saw sliced through the tense silence of Trauma Room 3. Marcus maintained a firm, unyielding wall with his broad shoulders, holding Daniel back. Daniel’s pristine demeanor had completely fractured into visceral, aggressive desperation.

"Get your hands off me!" Daniel snarled, his voice cracking with a mix of fury and fear. "I am her husband! You don't have permission to do this! I will sue this entire hospital! I'll have your medical licenses revoked by nightfall!"

"Sir, you need to step back to the waiting area immediately," Marcus replied, his voice unnervingly calm, tuned by years of de-escalating belligerent patients and violent family members. "You are interfering with emergency life-saving treatment."

I didn't lose another second. My hands, hardened by years of trauma response, guided the blade along the yellowed fiberglass. The saw's toothless blade vibrated violently against the hardened material without cutting soft tissue, but as the shell split wide, a cloud of foul, stagnant heat erupted from within.

The room was instantly overwhelmed by the stench of necrotizing fasciitis—flesh-eating bacteria—compounded by deep, decaying tissue. Nurse Sarah gagged behind her surgical mask, stepping back reflexively before forcing herself forward with a handful of sterile gauze.

"God in heaven," Marcus breathed, his eyes wide.

As I peeled back the soft padding beneath the fiberglass, the horror unveiled itself. It wasn't just a misaligned healing bone or a localized bed sore. Emily’s forearm was a nightmare of severe physical abuse disguised as a medical disaster.

Her wrist bore deep, jagged lacerations that didn't match a fall down the stairs—they were chemical burns and deep, binding grooves left by heavy industrial zip-ties. Beneath the cast, someone had intentionally shoved a rusted metal plate against her flesh, forcing it tight against her skin before wrapping the fiberglass over it. The pressure necrosis had completely killed off the upper layers of skin, turning them into black, leathery eschar. Infection was actively draining from open, green-tinged wounds, seeping into her bloodstream and driving her into profound septic shock.

"She didn't fall," I said, my voice dropping to an icy whisper. "This cast wasn't put on by a clinic. This was put on in a basement."

"Look at her radio-ulnar joints," Sarah whispered, pointing with a gloved finger. "That bone was smashed deliberately."

Daniel saw the cast fall open. He saw the black skin and the raw horror exposed to the bright fluorescent surgical lights. His eyes darted around the room like a trapped animal's.

"She did that to herself!" Daniel shrieked, making a desperate lunge toward the bed. "She's unstable! She's mentally ill! Don't listen to her!"

"Security!" I shouted into my intercom. "Code Violet in Trauma 3! Right now!"

Daniel didn't wait for the guards to arrive. Realizing he had lost control of the situation and that his crime was fully laid bare, he shoved Marcus backward with shocking strength. Marcus stumbled into an IV pole, crashing it to the floor. Daniel lunged toward Emily, reaching for her throat as if to silence her permanently.

"You ruin everything!" Daniel roared.

Before his fingers could graze her pale skin, I grabbed a heavy stainless-steel tray of surgical instruments and slammed it sideways into Daniel’s chest. The metallic clang echoed through the bay as he reeled back, gasping for air. Seconds later, three heavy-footed hospital security officers burst through the double doors, tackling Daniel to the floor.

They pinned his arms behind his back, forcing his pristine jacket into the spill of dirty coffee and sterile wash.

"Get him out of here and hold him for the police!" I commanded, my chest heaving. "Tell dispatch this is an attempted homicide and severe domestic abuse case!"

"You're lying!" Daniel screamed, his face smashed against the rubber floor tiles as he was forcefully handcuffed. "She's crazy! Emily, tell them! Tell them you fell!"

Emily didn't speak. She slowly closed her eyes, a single tear cutting a clean line through the grime on her pale cheek.

As security dragged the screaming man down the corridor, I turned back to Emily. Her vitals were plunging rapidly. Her blood pressure read 70 over 40. The sepsis was shutting down her organs.

"Sarah, call the OR. I need Dr. Vance down here immediately," I ordered, squeezing Emily’s clammy, non-affected hand. "Tell him we have an emergency arm-sparing debridement and compartment syndrome release. If we don't get her into surgery in ten minutes, she's going to die."

Emily’s fingers twitched faintly within mine. She opened her eyes slightly, looking up at me through a haze of agony and fever.

"Is he... gone?" she breathed, barely audible beneath the hiss of her oxygen mask.

"He's gone, Emily," I said softly, bending down close to her ear. "You're safe now. I promise you, he will never touch you again."

She let out a long, shuddering breath, and her monitor began to beep erratically as her body gave up its final desperate reserve of energy, slipping into unconsciousness.
